ASR Featured Form: Affidavit Form

Did you know that if your horse’s registration certificate has been lost or destroyed that you may apply for a duplicate registration certificate? The Registry offers the Affidavit form to be completed for lost or destroyed certificate replacement.  The affidavit will need to be completed, notarized, and submitted to the Registry along with four photos of the horse (front, rear, and both sides) and the $50.00 affidavit fee.
