The forms below can be completed electronically by saving the file and then submitting by email to registry@saddlebred.com or printed and faxed to (859) 259-1628 or mailed to ASHBA, 4083 Wing Commander Way, Suite 50, Lexington, KY 40511.

Online Forms are available to members and include Applications for Registration, Stallion Service Reports and DNA Kit ordering. Sign in to your member profile at the top right of the home page for access to online forms.

Authority Forms

• Statement of Signature Authority: To be used for any Recorded Owner that is an individual or a limited liability company, corporation, estate, trust, partnership (general or limited) (the “Entity”) to designate the individuals with authority to sign on behalf of the Entity

Breeding Forms

• Stallion Service Report: Deadline is January 15 of the year following the year during which the stallion is bred
• Stallion Authorization: Application for Breeder's Certificate and Stallion Service Report Authorization
• Notice of Breeding Lease Agreement: Use to notify the Registry of the ASHBA of lease of a horse for Breeding Purposes
• Notice of Castration Form: Use to report when a stallion has been gelded
• Owner's Statement of Frozen Semen: Use to notify the Registry of the ASHBA when current owner of stallion has frozen semen stored and that stallion has been gelded, or has died
• Statement of Retained Frozen Semen at Time of Sale of Stallion: Use to notify the Registry of the ASHBA that the stallion owner is retaining frozen semen with ownership rights upon the sale of this stallion
